Home Health services are skilled services that are usually provided on a visit basis, as opposed to hourly, for a short duration of time. These services are usually provided by a licensed and /or Medicare certified agency. Home Health services are provided by skilled professionals including nursing; physical, occupational and speech therapy; medical social work and home health aide.

Ensure that you use the self study tools and process to not only develop structure but also process for home health and hospice accreditation standards.

Ensure that you have forwarded a copy of your OASIS test transmission report (home health only) and your “855 review completed” letter from your fiscal intermediary. These documents must be on file with CHAP before we can schedule your site visit.

Ensure that you meet minimum census requirements and notify us. For deemed status, home health must have served 10 patients and have 7 active at the time of the site visit, and hospices must have served 5 and have 3 active patients. These are not required to be Medicare patients.

Our requirements for accreditation fall into 3 primary categories:

All of these requirements can be found in the Comprehensive Standards Manual for Home Care that you will receive free-of-charge upon receipt of your deposit.  You will receive both a hard copy print version and new e-dition of the manual approximately 3 weeks after your deposit is processed.
